#1. In order for a student to be eligible to participate in Crusader Athletics, an FHSAA Pre-Participation Physical (FORM EL2) must be completed and submitted to the Athletic Director (Richard LaBounty) or Athletic Secretary (Keith Haynes) at CHS. These must be completed prior to tryouts of the sport in which the student wishes to participate or prior to summer workouts. DO NOT TURN THIS FORM IN TO THE COACH. The 5-page form is available above and may be printed front and back. Please bring this to your physician, clinic, chiropractor as this is a newly updated EL2 form and they will probably not have any available.
#2. In addition, an online account entitled the ATHLETIC CLEARANCE ACCOUNT must be opened or updated every year. This will include necessary information from the student and the parent to complete including digital signatures for both. Click here for instructions on opening the account. There is a HELP button for assistance and an email for questions or problems. In addition, you may contact Mr. Haynes at khaynes@pensacolachs.org or at 850-436-6400 ext. 623.
While completing your Athletic Clearance Account, part of the account called FILES, will require you to upload the EL2 physical. The only page that needs to be uploaded is page four. If the student was sent to another physician to be cleared, page 5 would have been used and that must be uploaded as well. If you do not have the capability, you may email it to Mr. Haynes, and he will upload it. Please watch the video below for more information.
#3. Students must watch 3 required NFHS videos regarding concussions, cardiac arrest and heat related illness. Click here for directions on the NFHS Videos. If you already have an account, please use your current username and password to re-watch. If you do not, please create an account. Videos must be watched every school year. After each video, it will give you the opportunity to get a certificate. You may print these to retain for yourself. However, we no longer need to upload them to the account. By digitally signing the Consent Forms (Concussion, Cardiac Arrest & Heat Related) in the Signature Area of the online Athletic Clearance Account, the student and parent are confirming that the student has watched all 3 videos.
NOTE: Items #1, #2, and #3 are only good for 365 days before needing to be renewed. That is why we encourage everyone to come to the FREE PHYSICAL given every year. This year’s (Date TBA) physical will enable all students to play any sport the whole school year without worry of their physical expiring. You will be notified of the next date, place and time in May of 2024.
#4. If you are transferring into CHS from ANOTHER HIGH SCHOOL ONLY, you will also need to complete the Affidavit of Compliance (GA4). This must also be uploaded in the FILES section of the ATHLETIC CLEARANCE ACCOUNT along with the EL2 and Video Certificates.
